“Spick and span” means very clean. Originally it may have meant “brand new” but now anything can be spick and span, so long as there is not a speck of visible dirt on it.
「今から部屋の片づけを始めなさい。お祖母ちゃんが着くころにはピカピカに綺麗じゃないとダメよ！」
「Spick and span」は「とてもきれいな」という意味です。元々は「新しい」という意味らしいですが、全く汚れていないのなら古いものでも「spick and span」になれます。
